Open only to legal residents of the 50 United States and D.C. who are 18 years or older and physically located in the United States at time of entry. Void in Puerto Rico, U.S. territories and possessions, and where prohibited. Employees, officers, contractors, agents, and immediate family or household members of Midi Health, Mount Sinai Health System, USTA, US Open, promotion vendors, and their affiliates are not eligible.
To enter, complete the official entry form during the promotion period. Limit one entry per person. Entries must be complete, truthful, and submitted before the stated deadline. Void where prohibited.
Five winners will be selected by random drawing from all eligible entries. Each winner will receive one pair of U.S. Open tickets. Approximate retail value: $964 (August 30th match) and $988 (August 31st match) per pair. Total ARV: $4,868. Transportation, lodging, parking, meals, taxes, and all other expenses are not included and are the sole responsibility of the winner and guest.
Potential winners will be notified by email and must respond to Sponsor’s winner notification within 48 hours. If a potential winner does not respond by the deadline, is ineligible, cannot be contacted, fails to complete required documents, or declines the prize, the prize may be forfeited and Sponsor may select an alternate potential winner by random drawing from remaining eligible entries. Sponsor may use a shorter response deadline if required by event timing and stated in the winner notification.
Tickets will be delivered electronically using the ticket issuer’s designated transfer method. Winner and guest are responsible for having any device, account, app, email address, internet access, or other technology needed to receive, access, and use the tickets. Sponsor is not responsible for winner’s or guest’s inability to access or use tickets due to device, account, app, connectivity, or user error.
If a winner cannot reasonably access the electronic tickets, Sponsor may, in its sole discretion and if time permits, attempt to help with an alternative delivery method, but Sponsor is not required to provide any alternative prize, cash value, or substitute delivery method.
Winners are responsible for all applicable taxes. By entering, participants agree to the Official Rules, release the sponsor and promotion parties from liability, and consent to use of their name, likeness, city, and state for promotional purposes unless prohibited by law.

The Midi Health x Mount Sinai Health System US Open Ticket Sweepstakes (the “Sweepstakes”) is sponsored and administered by Midi Health, Inc. (“Sponsor” or “Midi”). Mount Sinai Health System is an exclusive health system partner in NYC.
The Sweepstakes is open only to legal residents of the 50 United States and D.C. who are 18 years of age or older and physically located in the United States at the time of entry. Void in Puerto Rico, U.S. territories and possessions, and where prohibited.
Limit one entry per person and per email address during the Promotion Period. Multiple entries submitted by the same person or email address will be void. Sponsor may disqualify any entrant who uses multiple email addresses, identities, automated methods, or other means to exceed the entry limit or interfere with the Sweepstakes.
Employees, officers, directors, contractors, agents, and representatives of Midi Health, Mount Sinai Health System, USTA, US Open, promotion vendors, and each of their respective affiliates, and immediate family or household members of the foregoing, are not eligible.
Existing patients may enter, but patient status may not be used to target, invite, prefer, weight, or select entrants or winners.
The Sweepstakes begins at 12:00 p.m. ET on July 27, 2026 and ends at 11:59 p.m. ET on August 17, 2026 (the “Promotion Period”). Sponsor’s computer is the official timekeeping device.
During the Promotion Period, visit pro.joinmidi.com/enter-to-win and complete the official entry form. Limit one entry per person/email address. Entries must be complete, truthful, and submitted during the Promotion Period.
No purchase, payment, appointment, consultation, referral, enrollment, or use of Midi Health or Mount Sinai Health System services is required to enter or win.
Five prizes will be awarded. Each winner will receive one pair of tickets to a US Open session selected by Sponsor. Precise tickets (location, seat number, etc.) will be awarded to winners randomly; winners are not able to select tickets. Approximate retail value: $964 (August 30th match) and $988 (August 31st match) per pair. Total ARV: $4,868.
Transportation, lodging, parking, meals, taxes, and all other expenses are not included and are the sole responsibility of the winner and guest. Prizes are non-transferable, non-refundable, and not redeemable for cash. Sponsor may substitute a prize of comparable value if a listed prize becomes unavailable, provided the substitution does not increase total ARV above $4,868 unless Sponsor completes any required registration and bonding analysis.
Sponsor will randomly select 5 potential winners from all eligible entries received. Odds of winning depend on the number of eligible entries received.
Sponsor will conduct the random drawing on or about August 19, 2026.
Potential winners will be notified by email. Potential winners must respond within the time stated in the notice, confirm eligibility, and complete any required release or verification documents. Failure to respond, failure to confirm eligibility, return of a notice as undeliverable, or noncompliance with these Official Rules may result in forfeiture and selection of an alternate winner.
Sponsor may use a 48-hour claim deadline for tickets, or another deadline stated in the winner notice.
Winners are responsible for all applicable federal, state, and local taxes and any other costs associated with prize acceptance or use.
Except where prohibited by law, acceptance of a prize constitutes permission for Sponsor, Mount Sinai Health System, and their designees to use winner’s name, city, state, likeness, and prize information for advertising, publicity, and promotional purposes without additional compensation.
Information submitted in connection with the Sweepstakes will be used to administer the Sweepstakes and as otherwise described in Sponsor’s privacy policy. Marketing consent is optional and is not required to enter or win.
Sponsor may disqualify any entrant who tampers with the entry process, violates these Official Rules, submits false information, uses automated entry methods, or acts in an unsportsmanlike or disruptive manner.
Sponsor is not responsible for lost, late, incomplete, inaccurate, misdirected, delayed, or undeliverable entries or notices, or for technical, network, platform, email, or other failures that may affect entry, notification, or administration.
By entering, entrants agree to release and hold harmless Midi Health, Mount Sinai Health System, USTA, US Open, promotion vendors, and each of their respective affiliates, officers, directors, employees, agents, and representatives from any claim, liability, loss, damage, or injury arising out of participation in the Sweepstakes or acceptance, use, or misuse of any prize.
Use of tickets is subject to all venue, ticket issuer, USTA, US Open, and event terms, policies, and restrictions. Event dates and times may change. Sponsor is not responsible for cancellation, postponement, delay, denial of entry, removal from the venue, or any ticket or venue restrictions.
